• If you have coverage for routine eye care, please let us know who to bill PRIOR to your exam.
• We participate in the plans listed below.
• Please become knowledgeable about your insurance coverage.
• Your insurance policy is an agreement between you and your insurance company.
• Our relationship is with you, NOT with your insurance company, so payment is ultimately your responsibility.
• Please understand that although an authorization may have been obtained from your insurance company, there are times when authorizations are given but benefits are not actually available.
• It is beyond the ability of Premier Family Eyecare to know if benefits are actually available.
• Any co-pays, deductibles and items not covered by insurance are your responsibility.
